Fenofibrate is used for the treatment of heart attack and heart stoke . this medicine
mechanism is lowering the blood LDL (Low density lipoprotein ) and triglycerides.

Fenofibrate Mechanism of action

Fenofibrate are comes from the fibrate groups. Chemical name of Fenofibrate is 2-[4-❲4-chlorobenzoyl❳ phenoxy]-2-methyl-propanoic acid.

This medicine are helps to activate the enzyme Lipoprotein lipase. This enzyme key role is
degradation of VLDL (Very low density Lipoprotein). So the thyroglobulin level in the blood
are ultimately decrease.

This whole process are continued by Peroxisome proliferator activated receptor πžͺ (PPARπžͺ).
This receptor PPARπžͺ is located in the liver. Which is a gene transcription regulating receptor.

When this receptor activate it increase the synthesis of lipoprotein lipase and fatty acid
oxidation. This medicine also decrease the hepatic thyroglobulin synthesis.

This medicine are helps lower thyroglobulin levels 20-50% by the increase HDL channel
10-15% and decrease 10-15% of LDL channel.

Generally free fatty acid are break down by 𝛃-Oxidation pathway to give the energy to
the body.

Apo A1 and Apo AII protein are helps to increase the HDL levels in the blood.
Apo CIII synthesis are reduce the  π›ƒ-Oxidation process resulting increase the free fatty
acid in the body. Which is harmful to the human health.

Fenofibrate help to stimulate the Apo AI and Apo AII for the synthesis of the more
friendly HDL in the body simultaneously decrease the production of Apo CIII to protecting
the process of 𝛃-Oxidation pathway.

Overall process helps to controlling the LDL, free fatty acid and TG in the body.

Pharmacokinetics of Fenofibrate

Fenofibrate is a prodrug of fenofibric acid . In the body fenofibrate is converted into
fenofibric acid by ester hydrolysis to show the pharmacological action.


This medicine is hydrophobic in nature so this medicine is not given through injectable form.
But it well absorbed by the gastrointestinal tract.

60% of the drug are eliminated by urine and 25% of this excreted by feces.
Plasma peak area occurred in the 6-8 hours after administration of the dose.
Absorption of this medicine is increase when this medicine is take along with food.

Fenofibric acid is metabolised by glucuronic acid conjugation not by cytochrome p450
enzyme.Same dose of this medicine works both the age group e.i younger patients and
older patients.

along with lipid lower diets or cholesterol free diets.

Excess body weight and excess intake of alcohol are importance factor of hypertriglyceridemia.

Dose for Primary Hypercholesterolemia and Mixed Dyslipidemia

For this condition Initial dose 160 mg per day

Dose for Severe hypertriglyceridemia

Initial dose for this condition is 54 to 160 mg per day. Maximum dose is 160 mg per day.

Some Warning

Those patients are suffer diabetes (specially older age group) or hyperthyroidism the risk of
skeletal muscle toxicity may appear.

There are various type of symptoms are shown like muscle tenderness or weakness ,
elevation of creatine phosphokinase levels ,muscle pain , malaise and fever shown.

When dose is increase 107 mg to 160 mg per day there are some adverse effect shown
in the liver function. Imidiate report side effects to doctor.
