Donepezil is the medicine that comes to the market name as Aricept treatment of Alzheimer’s disease .and it also uses for the treatment of dementia and confusion. Donepezil also increases the brain memory and intelligence and function of the brain.it mainly balancing and restoring the neurotransmitter in the brain.

Mechanism Action of Donepezil

Mechanism of donepezil actually not know clearly. but it may act as an anticholinesterase .
Donepezil is bind with cholinesterase reversibly.and increase acetylcholine.
Acetylcholine is very helped full to our body mainly to the brain .it helps to send the signal form one cell to another cell that’s the reason our brain working, .if neurotransmitter(acetylcholine) are not working properly then the body becomes paralyzes.
Donepezil is acting on the anticholinesterase to prevent the breakdown of acetylcholine to choline and acetic acid so acetylcholine sends the message to another neurotransmitter efficiently. Donepezil is a lipophilic drug it crosses the blood-brain barrio and it acts on the CNS more effectively .Donepezil is bind with cholinesterase binding site as competitively with acetylcholine 

Donepezil metabolism

Donepezil is metabolized by the enzyme Cytochrome Isoenzyme name 2D6 and 3A4.The rate of metabolism of donepezil is slow. Donepezil takes 3 to 4 hours to obtain the maximum plasma concentration. An oral dose of donepezil produces high plasma concentration also depends on the dosage of medicine.if the patients are taking the small dose frequently half-life of the donepezil is approx. 70 hours. Donepezil hydrochloride bound with plasma proteins aprox. 96 % and rest of all 28 % remove unchanged or uncover form the body. Donepezil is mainly excreated through the urine.
Donepezil is studied as radioactivity lable C14. Sex , smoking history and race have no clinically any effect to plasma the concentration of donepezil.

Donepezil Dosage

Donepezil is available in the market 5 mg, 10 mg, 20 mg
Mainly donepezil is given to the patient first 5 mg then gradually increases the dose per day. donepezil taking the best time is evening and morning.
To cure the patient for an intermediate state of Alzheimer disease the dose is given by the doctor 10 mg orally per day up to 4 to 6 weeks after completion of the initial dosage.
When a patient suffers severe Alzheimer’s disease dosage is given to the patient 20 mg per day for 3 months. 5 mg dose of donepezil is less incidence than 10 mg dosage of donepezil. But the adverse effect of donepezil is similar to 5 mg and 10 mg.

Before taking Donepezil to take some precaution

The patient must take some care which affects the heart disease , cardiac conduction (some muscle in heart cell are sending the signal to contract the heart), chronic obstructive pulmonary disease, asthma, severe cardiac arrhythmias and sick sinus syndrome(sinus dysfunction)
If any portion is taking NSAIDS or patient suffer the peptic ulcer disease high risk of gastrointestinal bleeding occurs. slow the heartbeat also seen in the patient .this type of symptoms are more when the dose of donepezil is increase 5 mg to 10 mg.
